Request Patient's Data

This component helps in requesting patient data.

To request a particular hospital data this form has to be filled

Enter the purpose for requesting the data, be precise with the purpose, as the hospitals would make decisions based on the purpose.

Enter the reason for requesting for the dataset.

Requests would be shown in the request tab as :

The download button would turn green, when the approval party has approved it, currently the requested party could delete the request.
